Black Spot on forehead of Scribble Psudotropheus

Not sure what is wrong with my fish but he has developed a black, somewhat darkish spot on his forehead. He is somewhat lethargic and tends to hang towards the top of the tank now. which he never did before. I have not medicated yet for fear of treating the wrong problem. Does anyone have any suggestions?

How often do you do water changes? Do you see him getting picked on?

I'd try a water change and see if it help. It could be simple "peppering" such is found on discus sometimes.
