dwarf frog planted tank help

i think mine is sick...does anyone know if planttbabs would be safe for one. i put the proper amount in and it is 11% nitrogen 4.9 ammoniacal nitrogen 6.1 nitrate nitrogen 15 phosphoric acid and 20 soluble potash. hes in a 10 gallon tank with a filter and he started getting really slow after i added these.

I don't know but if I ever added something and saw my fish etc acting weird right afterwards, my first reaction would be to do a massive water change.
