Help! Disease

I need help identifying the disease that has struck one of the fish in my tank:

Please see attached pic.

On the body of my swordtail male three separate white/grey spots appeared.
One of the head, one by the big fin on its back and one by the tail.

Please help me identify what can this be,

Looks like a small fungal/bacterial infection.

Melafix should help. Also, make sure you're doing weekly water changes and not overstocking.

If Melafix hasn't improved the condition in a week, something like Jungle Anti-Fungus should get rid of it.

Ahh looks like what lotus said as a small fungul bacterial infection. Two of my gouramis had the exact same bacterial infection in the exact same spot. I found out it was caused by my Synodontis biting and opening up a wound which was then infected. You might want to check you tank to see if this is happening.
