theres this little white worm looking thing sticking from his bottom fin.

none of the other fish have shown signs, they eat great.
should i treat the whole tank?

Yes, definitely treat the whole tank.

I've used something called Gel-Tex Ultra Cure Px for IPs, I soak the food in it, although they're supposed to eat it (none of my fish have). I haven't used it on discus, but I've fed food soaked in it to all kinds of fish, and even shrimp haven't been adversely affected by it.

Try to find a med that can be fed to the fish, as it's generally more effective than adding stuff to the water, assuming your fish are eating. There is a Jungle Anti Parasite food, but I haven't tried it.
