My tank has two vertical glass overflow boxes, built-in in the back corners.
On these overflow, there was no grate or any slots made. Fish were lost to getting into the overflow boxes and being overwhelmed by the whirling water. How I fixed it after the fact was the Gutter Guard by Home Depot. I cut two small sections and mounted them backwards, leaving the slots facing into the tank, but letting water exit the overflows. So far, so good. I had several other ideas, but this was readily available and all it took was aviation snips and a little work.
No fish in the overflows since. Problem solved.
