It sounds like what my tank looked like when I had a bacterial bloom. What size tank, how many/what kind of inhabitants, and what are your ammonia, nitrite and nitrate readings? Also, when you say "change water every week" are you doing a complete water change or just a partial? If you're doing a complete water change than the chances are more than likely you're having a constant bacterial bloom because the good bacteria is having to re-establish itself with every water change. Water changes should be 25-30% of your tanks total volume weekly, or more depending on your stocking level (or if you're keeping goldfish, which are notoriously dirty).

The tank is 42gal and I have only 4 tropical fish there (2 tigers, 2 angels (small size)). I started changing water once a week only for the last few weeks. before, I was doing it every 2 week. Water changes as always- 25%.
I tested ammonia, nitrate and nitrite- all readings are at the lowest level.
