Tetra died

I woke up this morning and one of my black skirts was stuck to my filter intake. I have absolutely no idea what happened. It aS fine last night and was chasing the others around like it usually did. I don't think it's my water, as it tested clean:

Ammonia- 0
Nitrites- 0
Nitrates- 25 (I always have trouble here, but it's a fish only tank)
Hardness- way high, but that's the way my local water is.
pH- 7-8

I do weekly water changes of about 20% and vacuum every two weeks and change about 40%. Any ideas or was this just an odd occurance?

I found one of my tetras dead last summer, of course it was THE one that always followed me around when I was walking by the tank and knew I was feeding him. No idea why, and his 2 (at the time) tank mates are still here to tell the tale. My water was fine and those 2 are still fine. Sad! They were very young, but not fresh from the pet store or anything. I was shocked that day.
