I have 3 catfish in the 40g. tank. One has fin rot. I treated it with Melafix for 8 days, then added carbon and after 10 days it came back. What would you recomend to do, which medication

Water parameters
0 nitrites, amonia
nitrates ~2
water changes every 4-5 days

So using Melafix helped it clear up? Usually the suggestion is to make sure and use the medication for longer than you think is necessary. If Melafix helped then I'd probably use Melafix again.

Also, I'd toss the carbon that you used to take the medication out. If any nasty bacteria were also absorbed then you dont want to release that back into the tank. Also think about anything that you put into the tank while the fish was affected, nets, test tubes, siphons etc and make sure to sterilize those because you might just re-infect the tank if you use them again after its cleared up.

I hear about people sometimes using Pimafix and Melafix with each other...you might try that? But if it worked before, I wouldn't hesitate to use the Melafix again.

It got better while using Melafix, but there was a hole remaining. I read that it can stay.

I did not know about sterilizing tubes and other stuff. I mean i cleaned it... I will defenetly try this. How is about gravel?

I am trying to find any information about treating both - Primafix and Melafix. should I add them alltoghether, or one day - melafix, second day - Primafix...

...I got up and read the label on both. Melafix says to use for 7 days, so I'd use it for 14. Pimafix looks like its only for fungal, not bacterial...so scratch that I don't think it'd help.
