It sounds like a fungal/bacterial infection. I'd get a combination med to put in there ASAP (although it would be better if you could treat him in a hospital tank).
Meds that have worked for me on similar diseases are AP Fungus Cure and Jungle Fungus Cure fizzy tabs.
