white haze in frog tank

I have a adf, and just moved him to a new tank because in the old one i was trying to start a live plant growing from a bulb. The water was so white and merky that you couldnt even see the plants. So i moved him into a newly cycled 10 gall. with the old filter, heater and thermometer, and a few gravels. Now its been about 2 weeks, and the problem is starting to come back. I thought it was just from the live plant, but there is no live plant anymore, so what could be causing this horrible white haze that i cant get rid of? ( I do a 25% wt. chg. every week).

after miss piggy died and i went back to feeding as normal, it completly cleared up all by itself, and 1 25% water change and syphone, it took about 2 weeks but its back to normal now. thank you for your suggestions and help, it is very much appriciated. Thank You lotus

Kermit was found doing a backbend at the bottom of the tank this morning. As far as i can tell, i didnt see any physical symptoms of anything. Kermit just started to shed for his first time about 3 days ago, and then it started looking a little slimey yesterday. He was also darting around alot yesterday and trying to hop off the top of the filter to escape. Hes never acted so stange and now i guess i know why. Bye Kermit, i love you and will miss you greatly.
